چکیده
In acute medical wards the use of comprehensive assessment techniques is mandatory [3]. The evolution of nurses’ role in assessment and care management of acute medical patients, mainly complex and frail, represents the basis to realising good governance of chronic disease in hospitals [4]. The problem is how to connect good management of difficult hospital discarge, and, in general, of complex patients with the management within the community: where are the ‘Intermediate care structures’ in the Italian National Health System? Where are the Managed Care organisations [5]?
